What Is a Nurse Practitioner?

A Nurse Practitioner, or NP, is a Registered Nurse with advanced education and clinical training. In Ontario, Nurse Practitioners are regulated healthcare professionals with an expanded scope of practice.

Within their individual competence and scope, Nurse Practitioners can:

  • Assess health concerns and symptoms
  • Diagnose illnesses and medical conditions
  • Prescribe medications
  • Order and interpret diagnostic tests
  • Provide treatment and follow-up care
  • Monitor acute and chronic health conditions
  • Make referrals and request specialist consultations
  • Provide health education and preventive care

Nurse Practitioners practise autonomously and are accountable for the care they provide. They may also collaborate with physicians, pharmacists, therapists, dietitians, and other healthcare professionals when appropriate.

The exact services available will depend on your health needs, your appointment format, and your Nurse Practitioner’s clinical assessment.

General Health Concerns

Our Nurse Practitioners can assess many common health concerns, including new symptoms, minor illnesses, pain, fatigue, sleep concerns, digestive symptoms, headaches, and changes in your general well-being.

Some symptoms require an in-person examination, diagnostic testing, or emergency assessment. Your Nurse Practitioner will advise you when virtual or clinic-based primary care is not sufficient.

Preventive Health and Wellness

Preventive care focuses on identifying health risks and supporting well-being before more serious concerns develop. Depending on your age, health history, and individual risk factors, care may include:

  • General health and lifestyle assessment
  • Blood-pressure monitoring
  • Health screening discussions
  • Sexual and reproductive health education
  • Review of family and personal medical history
  • Nutrition, sleep, and physical-activity guidance
  • Risk-factor identification
  • Age-appropriate preventive care planning

Chronic Condition Support

We may assist with the assessment, monitoring, and ongoing management of chronic health conditions, such as:

Your care plan may include symptom monitoring, medication management, diagnostic testing, lifestyle recommendations, referrals, or collaboration with other healthcare professionals.

Medication Review and Management

We will review your medications to help determine whether they remain appropriate, effective, and safe.

Medication-related appointments may include:

We can’t guarantee a prescription or renewal. Medication decisions are based on your Nurse Practitioner’s assessment, professional judgment, access to relevant medical information, and applicable prescribing requirements.

Mental Health Support

Physical and mental health are closely connected. Our Nurse Practitioners can assess certain mental health concerns, discuss treatment options, review medications, and help determine whether additional psychological, psychiatric, or emergency support is needed.

As part of Positive Mind Wellness’s multidisciplinary environment, primary care may be coordinated with psychotherapy, psychological services, nutritional counselling, and other wellness services – with your knowledge and consent.

Diagnostic Tests and Referrals

When clinically appropriate, we may order and interpret diagnostic tests or request consultation from another healthcare professional.

This may include:

  • Laboratory testing
  • Diagnostic imaging
  • Specialist consultation
  • Allied healthcare referrals
  • Follow-up assessment

The availability, acceptance, coverage, and wait time associated with a test or referral are determined by the receiving laboratory, imaging facility, specialist, healthcare organization, and applicable provincial requirements.

Women’s and Reproductive Health

Your primary care may include support related to:

  • Menstrual concerns
  • Contraception
  • Sexual health
  • Fertility and preconception health
  • Pregnancy-related questions
  • Postpartum health
  • Perimenopause and menopause
  • General reproductive wellness

Some examinations, procedures, pregnancy concerns, or reproductive health needs may require in-person care or referral to another provider.

What to Expect From Primary Care

Your first appointment allows your Nurse Practitioner to develop a detailed understanding of your health. Depending on your needs, the assessment may include:

After completing the assessment, your Nurse Practitioner will discuss their findings, answer your questions, and recommend next steps.

Follow-up appointments are intended to address an established concern, review progress, discuss results, monitor medication, or make appropriate adjustments to an existing care plan.

A 15-minute appointment is best suited to one focused or straightforward concern. If you have multiple concerns, a complicated medical history, or require a more detailed assessment, please book a 30-minute appointment.

Your care plan may include treatment, medication, diagnostic testing, health education, monitoring, referral, or collaboration with another healthcare professional.

Recommendations are based on your individual presentation. We cannot guarantee a specific prescription, test, diagnosis, referral, or treatment before completing an assessment.

We don’t include any additional costs for medication, forms, external testing, medical records, or services delivered by another provider or facility unless we expressly state otherwise. Contact your insurance provider directly to determine whether any portion of your appointment may be reimbursed.

Preparing for Your Appointment

To help your Nurse Practitioner understand your health needs, please have the following available when relevant:

  • A list of current medications and supplements
  • Information about medication allergies
  • Relevant medical records
  • Recent laboratory or imaging results
  • Hospital or specialist reports
  • Home blood-pressure or blood-sugar readings
  • A list of your main questions or concerns
  • Contact information for your existing healthcare providers

If you have several concerns, identify the most important ones at the beginning of your appointment. Your Nurse Practitioner will address as much as can be safely managed within the scheduled time and may recommend another appointment for additional concerns.

Collaborative, Whole-Person Healthcare

Positive Mind Wellness is a multidisciplinary clinic. With your consent, our Nurse Practitioners may collaborate with other members of your care team, including:

  • Psychologists
  • Psychotherapists
  • Registered Dietitians
  • Registered Acupuncturists
  • Registered Massage Therapists
  • Physicians, pharmacists, and specialists
  • Other regulated healthcare professionals

This collaborative approach helps us consider how different aspects of your health may interact while ensuring that each professional works within their own scope of practice.

If you already have a family physician or another primary care provider, our services can supplement your existing care when appropriate. Sharing relevant records between your providers can help reduce duplicated testing and support safer continuity of your care.

When Primary Care Is Not Appropriate

Positive Mind Wellness is not an emergency department or crisis service.

Call 911 or go to the nearest emergency department if you experience:

  • Chest pain or severe difficulty breathing
  • Signs of a stroke
  • Severe bleeding
  • Loss of consciousness
  • A serious allergic reaction
  • A major injury
  • Sudden or rapidly worsening severe symptoms
  • Immediate danger to yourself or another person

Virtual appointments are not appropriate for every health concern. If a physical examination, urgent testing, or emergency intervention is required, your Nurse Practitioner will direct you to an appropriate in-person service.

You generally don’t need a referral to book a private appointment with a Nurse Practitioner.

Yes. Ontario Nurse Practitioners have advanced legal authority to diagnose illnesses and medical conditions within their knowledge, skill, judgment, and area of practice.

Nurse Practitioners can prescribe medications within their scope and competence. However, a prescription or renewal is never automatic and will be provided only when your Nurse Practitioner determines it is clinically appropriate and safe.

Nurse Practitioners can order and interpret many diagnostic tests. Whether a particular test is appropriate will depend on your assessment. Coverage and acceptance can also vary by test, facility, and provincial program.

Yes. However, it is important to tell your Nurse Practitioner about your existing providers and treatments. With your consent, we may share relevant information to support continuity and reduce the risk of duplicated or conflicting care.
